Závěr Vrozené poruchy imunitního systému představují stále se rozšiřující skupinu onemocnění, u které se díky pokrokům v objasňování jejich patogeneze daří také zvyšovat množství diagnostických a terapeutických přístupů. Velkým pokrokem na poli PID/IEIs v České republice je zavedení celoplošného screeningu SCID na počátku roku 2024. Rozvíjí se také léčebné možnosti, mezi které patří stále častější využívání biologické terapie pomocí monoklonálních protilátek a tzv. malých molekul. Pokroky jsou zaznamenávány také v možnostech genové terapie. S postupujícím poznáním však přichází i nové výzvy týkající se diagnostických, terapeutických i etických aspektů, se kterými se budeme muset v budoucnosti vyrovnat. Největší limitací širšího používání nových léčivých přístupů je kromě jejich ceny také fakt, že je zatím k dispozici jen velmi málo randomizovaných placebem kontrolovaných studií. V budoucnosti se snad dočkáme prospektivních klinických studií na ideálně co největším počtu pacientů s konkrétní diagnózou tak, aby mohla být pro jednotlivá vzácná onemocnění připravena jasná doporučení a postupy stran jejich diagnostiky i léčby.
