Introduction: Diabetic foot syndrome is often presented as a skin lesion in diabetics. The aim of our study was to analyse foot ulcerations in diabetics, together with ethiopatogenesis, location and grade of impairment. We assessed the relation between BNP levels and some echocardiographic parameters of systolic and diastolic function of the left ventricle in 49 patients (mean age 69.39 ± 8.47 years) with chronic kidney disease in different stages of chronic renal failure according to K/DOQI and in 45 subjects (mean age 52.6 ± 14.85 years) on dialysis. Gastroesophageal reflux disease is defined as a disorder where reflux of the stomach content is bothersome and/or brings about complications. A condition, where reflux symptoms persist or new occur and oesophagitis healing fails to take place despite maximum treatment, is classified as refractory gastroesophageal reflux disease. A motility disorder rather than acid hypersecretion is the most important cause of gastroesophageal reflux disease. Prokinetic agents are thus the most logical treatment choice for gastroesophageal reflux disease, although proton pump inhibitors or, alternatively, a combination of these two therapeutic groups are the most effective.

Head-up tilt test - do we really know all about it?

Head-up tilt test (HUTT) falls within the competence of functional cardiological examination. Most often it is being used for the diagnostical approach to possibly neurally-mediated syncope, less is known about the possibility of using tilt table test in the treatment of vasovagal syncope. The main goal of this article is to make clear the principles, appropriate indication, performance and subsumption of HUTT in the algorithm of syncope investigation. Moreover it will be marginally mentioned how HUTT can be used in predicting cardiovascular risk of patients with coronary artery disease.

Gastric neuroendocrine tumors

The aim of the article is to present the state-of-the art in terms of diagnosis and therapy of these rare, usually biologically inactive neuroendocrine tumors, being classified into three main types. They differ from each other according to the conditions of their origin, biological properties and management. Recent classification according to WHO and ENETS (European Neuroendocrine Tumor Society) and proposal of TNM classification are presented. Diagnostic methods and therapeutical procedures, including experimental approaches, are outlined.

IgA pemphigus, resembling subcorneal pustulous dermatosis, represents a rare complication of IgA type monoclonal gammopathy. The patient dates the onset of initial symptoms of vesicular-bullous disease to 1990. She was first examined at our clinic in 2001 with the following conclusion "type IgA monoclonal gammopathy of unknown significance". A 24-year-old man presented with cough, sore throat, fever, maculopapulous exanthema, pericardial and pleural effusion. Despite extensive evaluation neither infectious, autoimmune, hematological nor oncological disorders were revealed. Broad spectrum antibiotic and subsequently corticosteroid treatment failed to resolve the symptoms.
