Oxidative stress in patients after kidney transplantation

Successfull kidney transplantation (KT) leads not only to normalization of renal excretory function, but also to modification of the metabolic and endocrine kidney function. The most common cause of morbidity and mortality in patients after KT are cardiovascular diseases which development is also associated with oxidative stress (OS). KT and the post-transplantation period are associated with increased OS that could gradually decrease. Some immunosuppressive drugs also contribute to increase of OS, especially compounds from a group of calcineurin inhibitors and thus indirectly contribute to increased risk of cardiovascular complications.

Castleman's disease (also called angiofollicular lymph node hyperplasia) can take two forms with different prognosis: the localized form can usually be treated by a surgical intervention and has therefore a favourable prognosis. On the other hand, the multicentric form has an unfavourable prognosis and requires systemic treatment. Acquired haemophilia A is a rare form of acquired coagulopathy caused by autoantibodies against coagulation factor VIII. It is characterized by major bleeding complications often life-threatening. An estimated incidence is about 1-2 cases per million inhabitants per year. The authors report case history of acquired haemophilia in 85 year old woman. Bleeding complications were well controled by bypassing agents. Inhibitor eradication was successful after the use of second line agent rituximab.

Lung metastases as the first manifestations of occult medullary microcarcinoma

The article is concerned with medullary microcarcinoma of the thyroid. Similarly to medullary macrocarcinoma, this may metastasize to distant sites. Reported is a case of a 54-year-old male who had suffered from chest tightness and dry irritating cough. Chest X-ray showed small nodules with poorly-defined borders of unknown etiology. Use of vildagliptin from an internal disease specialist's point of view

Dipeptidyl peptidase 4 inhibitors (DPP-4) are used as an oral hypoglycaemic agent in Type-2 diabetic patients. From a clinical point of view the most important advantages of this preparation are improved diabetes compensation, significant reduction of hypoglycaemia risk compared with sulfonylurea derivatives, neutral weight profile and good GIT tolerance. Vildagliptin is a molecule from the group of DPP-4 inhibitors which is recently used in internal outpatient care.
