[Effect of vitamin E therapy on progression of diabetic nephropathy].

E Hirnerová, B Krahulec, L Strbová, A Stecová, J Dekrét, A Hájovská

Vnitr Lek 2003, 49(7):529-534

Pathogenesis of diabetic nephropathy, which belongs to most serious microangiopathic complications of diabetes, is still not completely clear. Thromboxan A2 and increased oxidation stress are new factors apparently associated with pathogenesis of diabetic nephropathy. It was the aim of the contribution to verify the participation of thromboxan A2 and oxidation stress in the pathogenesis of diabetic nephropathy, as well as to follow the effects of treatment with vitamin E on its progression. In 19 diabetic subjects with microalbuminemia (MA) (age 55.2 +/- 7.6 years), 10 diabetic subjects with normoalbuminemia (NA) (age 54.4 +/- 6.1 years) and in 10...

[Microvascular reactivity in type 2 diabetes mellitus and its relation to IGF-I and its binding proteins].

M Krsek, M Prázný, J Skrha, V Justová, Z Lacinová, T Haas

Vnitr Lek 2003, 49(7):535-540

BACKGROUND AND AIM: System of insulin-like growth factor-I (IGF-I) and its binding proteins (IGFBP) may be involved in the pathogenesis of vascular damage in Type 2 diabetes. Aim of the study was to analyse the relationship between this system and microvascular reactivity in Type 2 diabetes as measured by laser-Doppler flowmetry.METHODS AND RESULTS: Thirteen Type 2 diabetic patients (8 women and 5 men) with microangiopathy and fifteen healthy subjects (8 women and 7 men) were examined clinically, underwent laser-Doppler flowmetry and intima-media thickness measurements. Fasting serum levels of IGF-I, IGFBP and lipids were examined. Percentage perfusion...

[Comparison of procalcitonin, interleukin-6 and C-reactive protein in the differential diagnosis of patients with sepsis syndrome in intensive care units].

M Průcha, I Herold, R Zazula, L Dubská, B Kavka, M Dostál

Vnitr Lek 2003, 49(7):541-547

UNLABELLED: One of the most difficult tasks in differential diagnosis of patients with septic syndrome at the Intensive Care Units is to differentiate between infection and non-infection etiology of this syndrome. In the last years, new parameters have played an important role in this area--C-reactive protein, Interleukin-6 and procalcitonin.THE AIM: Of the investigation was to compare these three parameters in differential diagnosis of the septic syndrome. THE COHORT AND METHODS: The authors examined 56 patients (17 women and 39 men, mean age being 43 and 51 years, respectively) hospitalized at the Intensive Care Units who corresponded to the...

[What is the health status of cardiology health care personnel in 2001?].

E Sovová

Vnitr Lek 2003, 49(7):548-550

The prevention of cardiovascular diseases belongs to priorities of the Czech public health care. Medical personnel should therefore set the example in the healthy way of life. At the meeting of the Working Group for Arrhythmia and Permanent Cardiostimulation at the Czech Cardiological Society, a questionnaire-based investigation for the evaluation of basic risk factors in workers engaged in cardiology (physicians, nurses, technicians) was performed. Our employees represent a paragon for the population in terms of motion activities and smoking in men. All of them also evaluate their subjective health better than the general population does. However,...

[Relation of natriuretic peptides (BNP, NT-proBNP) to total mortality. A 4-year follow-up of patients].

T Kluh, A Jabor, M Pavlisová, J Povolný

Vnitr Lek 2003, 49(7):551-554

The paper deals with a group of 157 patients followed for the period of four years in view of the relation of concentrations of BNP or NT-proBNP to overall mortality. In the introduction the authors describe their experience with natriuretic peptides; their own data follow. The Kaplan-Meier analysis of probability of survival proved to possess a strong predictive efficiency of the concentrations of BNP or NT-proBNP for overall mortality of the observed cohort.

[Occurrence of chlamydia infection in relation to lipidemia indicators in the etiology of unstable angina pectoris].

K Zeman, L Pospísil, Z Medková, J Canderle

Vnitr Lek 2003, 49(7):555-558

The presence of Chlamydia pneumoniae infection was examined in 66 patients with unstable angina pectoris (UAP), 155 patients with acute myocardial infarction (AIM) and 112 controls without signs of a heart disease. Besides evaluation of anamnestic data, ECG and coronarographic examination, serologic examination of C. pneumoniae by the microfluorescent method anti-MOMP and ELISA of anti-LPS of globulin IgA and IgG serum classes in every patient was performed. Moreover, in patients with UAP, routine biochemical methods for the detection of total cholesterol levels and its lipoprotein fractions LDL, HDL and triacylglycerols were used. The levels of anti-MOMP...

[Molecular diagnosis in pancreatic carcinoma].

P Dítě, Z Lukás, M Hermanová, I Kroupová, Z Kleibl, J Brázdil, M Pazourková, Z Novotný, M Růzicka

Vnitr Lek 2003, 49(7):559-562

Present diagnostic possibilities virtually do not make it possible to diagnose early stages of pancreatic cancer. Likewise, it is very difficult to differentiate between pancreatic cancer and chronic pancreatitis. The methods of visualization are insufficiently sensitive and the determination of certain genes could enrich our diagnostic opportunities. Considerable attention in this direction has been devoted to the determination and evaluation of the presence of oncogene K-ras. Our initial experience with the determination of K-ras in preparations from patients with pancreatic cancer or with chronic pancreatitis confirmed that K-ras in an oncomarker...

[Color Doppler methods and their diagnostic significance].

I Hrazdira, E Kotulánová, V Marysková

Vnitr Lek 2003, 49(7):563-566

Colour Doppler methods represent an important progress in diagnostics of vascular diseases. Conventional spectral and colour imaging of blood flow velocity have been during last years completed by new modalities, such as power Doppler, application of echocontrast agents, contrast harmonic imaging and tissue Doppler imaging. These new methods have improved diagnostics in several ways, especially in identification of small vessels, in detection of low flows and in perfusion assessment.

[Fibrillary immunotactoid glomerulonephritis--an unusual cause of irreversible kidney failure].

A Zharfbin, K Husek, D Sobotová

Vnitr Lek 2003, 49(7):567-571

Fibrillary-immunotactoid glomerulopathy (FITGP) is a rare cause of nephrotic syndrome. In this patient, male 65-years-old, respectively, fibrillary glomerulonefritis (FGN) was diagnosed by percutaneous renal biopsy. Clinically, the disease manifested with long term proteinuria and nephrotic syndrome without any associated systemic disorder. Histologically, glomeruli showed deposition of PAS+, fuchsinophilic and Orange G+ material in the mesangium and basal membranes. Strong granular immunofluorescent IgG, C3, kappa and lambda light chains deposition was present in the mesangium. Electronmicroscopically, depositions of fibrillary material in the expanded...

[Hereditary pheochromocytoma--a family affected by von Hippel-Lindau disease].

J Mertl, A Rovný, A Krepelová, J Prásek

Vnitr Lek 2003, 49(7):572-575

The authors present a case of a 37 year old male (proband) with a 13 year history of progressive sight impairment leading to blindness and a 4 year history of a mild hypertension. He was incidentally found to have large adrenal tumors after an ultrasound kidney examination. The tumors were confirmed with CT scan and magnetic resonance imaging. A bilateral pheochromocytoma was biochemically demonstrated and successfully removed. The eye diagnosis of angiomatosis retinae von Hippel-Lindau was ascertained after a search of the patient files in other medical departments, which led to a family screening. Proband's brother, having hypertension and a history...

[Initial experience with treatment of chronic hepatitis C in HIV-positive patients].

P Husa, S Snopková, P Chalupa

Vnitr Lek 2003, 49(7):576-579

A 30 years old man originating from Ukraine was infected by the human immunodeficiency virus (HIV) and virus of hepatitis C (HCV) due to injection administration of drugs of abuse in his own country before coming to Czech Republic. He was infected by genotype 3 of HCV and the infection became chronic. Under the influence of a three-combination anti-retrovirus therapy his conditions related to HIV infection became stable and it proved to be possible to apply a combined treatment by alpha-interferon and ribavirin at commonly used doses for the period of 12 months. In the course of therapy the HCV nucleic acid (HCV RNA) disappeared from serum and serum...
