Smoking habits of female and male doctors compared with smoking in the population of the Czech Republic. Comparison with the situation in the year 1991
J. Widimský, Z. Škodová, T. Pecka
Vnitr Lek 2005, 51(3):286-290
In the year 1992 we published the analysis of smoking habits of female and male doctors comparing the occurrence of smoking with the situation in the Czech population of corresponding age monitored within the study MONICA. The aim of this presentation is the analysis of the situation in the year 2003 and its comparison with the situation 12 years ago. Anonymous questionnaire survey was performed in the year 2003 on the occasion of training proceedings given by the Institute for further education of health care professionals (IPVZ). We analysed answers from 470 informants. The principal finding of our study 12 years ago was the fact that the occurrence...
Chronic critical limb ischaemia - distal revascularisation vs distal revascularisation with free muscular transfer
B. Zálešák, P. Tošenovský, I. Čižmář, J. Zapletalová, M. Šimek
Vnitr Lek 2005, 51(3):292-298
The study compares treatment results in two groups of patients with critical limb ischaemia:the group A - 21 patients treated only with the pedal bypass is compared with the group B - 17 patients treated with the pedal bypass and free microsurgical muscular transfer covered with dermoepidermal graft. Primary/secondary one-year patency in the first group was 64.7/82.4% and 54.5/81.8% in the second group, respectively. Evaluation of the patency at the end of the study by means of the survival analysis together with construction of Kaplan-Meier survival curves and long-rank test did not prove significantly relevant difference between the two...
The causes of acute intoxications in the course of one year admitted to intensive metabolic care unit
J. Charvát, T. Vaněček, M. Souček, Z. Baraníková, S. Kozáková
Vnitr Lek 2005, 51(3):299-303
We present the survey of the patients admitted to Intensive metabolic care unit (IMCU) for acute intoxication during 1 year - 2002. During 2002 we have admitted altogether 677 patients to IMCU. Out of them 213 (31.5%) were admitted for acute intoxication, 100 men of the average age 36.8 ± 11.8 years and 113 women of the average age 41.5 ± 14.0 years. 6 patients were admitted twice for the reccurent intoxication. The most common cause for acute intoxication was suicidial attempt which was discovered in 149 cases of intoxication (68.1%), followed by toxicomania in 12.8%, alcohol intoxication in chronic alcoholics in 13.2%, accidentally...
Anticipated costs on treatment of diabetes mellitus and its late complications in CR - preliminary study
D. Bartášková, R. Kožnarová, M. Kvapil
Vnitr Lek 2005, 51(3):304-313
The cost of one-year treatment of diabetes type 1 as well as diabetes type 2 in Czech Republic in 2002 is assessed in this article. An estimated cost of treatment of late diabetes complications is also presented. This is an inicial study, which aim is to make an economic model of diabetes treatment. Direct medical charges were estimated only. We have assessed cost of following diabetes treatment methods: diet, PAD, PAD + insulin, insulin. The cost of following late diabetes complications was estimated: diabetic retinopathy, diabetic nephropathy, diabetic food, amputation, blindness, transplantation, and hemodialysis. The cost of diabetic neuropathy...

Diabetic cardiomyopathy
J. Dúbrava
Vnitr Lek 2005, 51(3):314-319
Diabetic cardiomyopathy (CMP) is defined as heart muscle disease specifically due to diabetes mellitus, distinct from coronary atherosclerosis and arterial hypertension, characterized by systolic and/or diastolic left ventricular (LV) dysfunction. Diagnosis of diabetic CMP requires exclusion of all other potential causes of LV dysfunction. Its pathogenesis is not definitively known. According to the current knowledge the decisive factor is a loss of cardiomyocytes due to increased apoptosis. Dominant pathological findings are interstitial fibrosis with collagen accumulation and PAS positivity and arteriolar hyalinization. Neither evidence of large-vessel...
Multi-component automated collection - novel tendency in blood donating
R. Procházková
Vnitr Lek 2005, 51(3):320-326
Multi-component blood collection represents a new tendency in transfusiology. Modern blood separators allows concomitant collection of erythrocytes, thrombocytes or plasma in various combinations from one blood donor in one session, with a result of high quality standardised transfusion preparations in the final form. The method represents the alternative to full blood donating, brings advantages for patients and transfusion sites. An application of more transfusion preparations from one donor decreases risks of haemotherapy in poly-transfusion patients, the method brings flexibility and rational utilisation of donor base for transfusion sites. Currently...
Hyperphosphataemia as an important complication of chronic renal insufficiency and chronic renal failure, part 1 - etiopathogenesis, consequences and diagnostics
J. Smržová
Vnitr Lek 2005, 51(3):327-335
Abnormalities of calcium-phosphate metabolism start developing as early as in mild to moderate renal insufficiency when the glomerular filtration rate drops to 1 ml/s. As a result, the abnormalities can be found in up to 5% of population. The tendency towards the phosphate retention initiates a complicated cascade of disorders that ends in secondary hyperparathyroidism, renal bone disease and calcium-phosphate metabolism disturbances. The whole process leads to extraosseal calcifications, accelerated atherosclerosis and rise in mortality. Understanding pathogenetic mechanisms of calcium-phosphate metabolism disturbances well is vital for successful...
Hyperphosphataemia as an important complication of chronic renal insufficiency and chronic renal failure, part 2 - treatment
J. Smržová
Vnitr Lek 2005, 51(3):337-344
Hyperphosphataemia is a commonly found complication of severe renal insufficiency. However, minor disturbances of calcium-phosphate metabolism are encountered in mild to moderate renal insufficiency and, as a result, can be found in up to 5% of population. They lead not only to renal bone disease but also to extraosseal calcifications (including the vascular ones) and increase in mortality rate. Their impact on the patients' quality of life is very significant, too. The treatment of hyperphosphataemia is closely associated with the therapy of renal bone disease. It consists in dietary restrictions, the use of a constantly widening range of phosphate...
Novel findings on iron metabolism
M. Šimek
Vnitr Lek 2005, 51(3):345-353
The article comprehensively summarises novel findings on iron metabolism. Iron resorption by the cell, by the erythrocytes and circulation in the body with further utilisation for the body needs is reviewed step by step. For proper understanding to iron metabolism disorders it is important to understand not only to individual processes in iron processing but also to their regulatory mechanisms that effectively keep iron homeostasis in the body.

Treatment of elderly patients with co-infection with hepatitis B and C viruses
J. Stránský
Vnitr Lek 2005, 51(3):354-358
A co-infection with hepatitis B (HBV) virus and hepatitis C (HCV) virus is not anyway a rare disease. Acute hepatitis caused by HBV and HCV co-infection occurs in about 5 % cases, predominantly in IV drug addicts, and it can reach 15-20 % in chronic hepatitis. It is impossible to find out retrospectively in older cases of chronic hepatitis, whether hepatitis experienced originated as a co-infection with both viruses or the super-infection with HBV or HCV occurred in the next course, but it is unsubstantial for antiviral treatment indication. Elderly patients with chronic hepatitis B and C over 60 years old belong among a little studied population which...
