Antiplatelet therapy by acetylsalicylic acid (ASA, aspirin) provided pivotal advances in the prevention and treatment of organovascular (angiovascular, cardiovascular, cerebrovascular, extremitovascular, renovascular, genitovascular, mesenteriointestinokolonovascular, bronchopulmovascular, oculovascular, otovascular and other) arterial ischemic diseases. Currently available antiplatelet drugs have some limitations which might be overcomed by improved dosing regimens, use of combination of agents affecting different platelet functions and, in particular, by the new antiplatelet drugs (new arterial antithrombotics) with distinct pharmacodynamic properties...

Long-acting insulins in the treatment of type 2 diabetes and their position in the current treatment algorithm
Martin Haluzík
Vnitr Lek 2017, 63(2):87-92 | DOI: 10.36290/vnl.2017.019
Insulin therapy has been for many years an inseparable part of the treatment of patients with type 2 diabetes, in particular those with longer diabetes duration. Current national and international guidelines list insulin treatment as a possible second choice therapy in patient with unsatisfactory glucose control on monotherapy with metformin. In reality, insulin therapy is often initiated later than it optimally should be. The reasons include among others the fear of patients and sometimes also of physicians from the side effects of insulin. Even though the options of antidiabetic treatment has been diversified by the addition of novel groups of antidiabetics...
Acute kidney injury: a current comprehensive overview
Štefan Sotak
Vnitr Lek 2017, 63(2):93-97 | DOI: 10.36290/vnl.2017.020
Acute kidney injury (AKI) is a syndrome characterized by the rapid loss of the glomerular filtration (GF) with consecutive retention of nitrogen-containing substances. It includes conditions from mild injury with slight metabolic derangements to oligoanuric failure with life-threatening metabolic disruption. 130 subjects with this renal injury were hospitalized at the Ist Department of Internal Medicine of the University Hospital of Louis Pasteur and the Medical Faculty of the University of Pavol Jozef Šafárik in Košice in the Slovak Republic in the years 2013, 2014 and 2015. It is resulted from the analysis of patients that they were mainly...
Autoantibodies in systemic connective tissue disease and ANCA-associated vasculitis, their relationship to interstitial lung diseases and prognosis
Martina Doubková, Jana Pokorná
Vnitr Lek 2017, 63(2):98-106 | DOI: 10.36290/vnl.2017.021
Lung disease, including interstitial lung disease (ILD), is a frequent complication of systemic connective tissue disorders (CTD) and ANCA (anti-neutrophil cytoplasmic antibody) associated vasculitis (AAV). Pulmonary manifestations are prognostic factor of CTDs and vasculitis. Autoantibodies assessment is a part of differential diagnosis algorithm of lung diseases. Autoantibodies importance is mainly clinical-diagnostic. Using detection of some autoantibodies it is possible to determine prognosis of lung involvement, especially in CTDs.
An assessment of VO2 kinetics in the recovery phase of cardiopulmonary exercise test in patients with heart disease - importance and classification
Leona Mífková, František Várnay, Pavel Homolka, Petr Dobšák
Vnitr Lek 2017, 63(2):107-113 | DOI: 10.36290/vnl.2017.022
The article reviews the course of VO2 in the phase of recovery kinetics of oxygen uptake (VO2rec) cardiopulmonary exercise test. VO2rec kinetics cannot be completely defined using a single parameter. The article describes the three most frequently used methods: %VO2-REC2, T1/2 VO2rec and TC VO2, and provides detailed procedures of their analysis. The three aforementioned parameters are also at the heart of our classification of VO2rec. The method %VO2-REC2 is an essential one, in our view, since its prognostic significance is proven and it...
Functional impact of hERG: from physiological role to target of anticancer therapy
Júlia Šatková, Markéta Bébarová
Vnitr Lek 2017, 63(2):114-123 | DOI: 10.36290/vnl.2017.023
The human ether-à-go-go related gene (hERG; officially designated as KCNH2) encodes the structure of protein forming α-subunit of voltage-gated ion channel which conducts the rapid component of delayed rectifier K+ current (IKr). This current plays an important role namely in the cardiac repolarization. Mutations in hERG result in inherited arrhythmogenic syndromes characterized by a lenghtening or shortening of QT interval on the electrocardiogram and by an increased occurrence of life-threatening arrhythmias. This review also introduces hERG channels as a part of regulatory...

Conservative treatment of late aortic graft infection
Peter Baľa, Kamil Zeman, Renata Kološová, Ivo Pavlas, Dušan Kučera
Vnitr Lek 2017, 63(2):133-137 | DOI: 10.36290/vnl.2017.025
Aortic graft infection is one of the most serious complications of aortovascular surgery. Diagnosis is based on clinical and radiologic findings. The emphasis is put on early diagnosis and its impact on prognosis of patients. The management should be individualized. The gold standard is surgical treatment with complete excision of the infected foreign material with debridement of the surrounding tissue and repair of vascular continuity by extra-anatomic bypass or in situ bypass with autologous femoral vein or cryopreserved arterial allografts. In stable patients with high risk perioperative mortality a conservative strategy may be an alternative approach....
Parathyroid cancer

Vnitr Lek 2017, 63(2):139-144 | DOI: 10.36290/vnl.2017.026
Parathyroid cancer is a rare endocrine malignancy, representing less than 1 % of all cases of primary hyperparathyroidism. The exact etiology of the disease remains unknown. Known risk factors include neck irradiation, end stage renal failure, genetic factors, particularly the the HPRT2/CDCT73 gene mutation. The clinical picture is often indolent, yet progressive with a trend of local invasion and metastasis formation in advanced disease. The clinical picture includes symptoms of severe and resistant hypercalcemia, requiring intensive therapy often with the need of dialysis. Radical surgery is the mainstay of the parathyroid cancer treatment....
