Vnitr Lek 2012, 58(11):817-824
Aim:To determine bone mineral density (BMD) in nephrology patients at the start of haemodialysis therapy and its dependence on some laboratory and clinical characteristics of the study set. Methods:There were 73 newly haemodialyzed patients accepted in the 3 months period from the beginning of the chronic haemodialysis program. Each patient underwent measurements BMD with DXA method in the area of lumbar spine and the left hip. Ca, P and parathormone values were measured once per month during 3 months before BMD determination. 25-OH vitamin D, estradiol and blood pH were determined only once before the densitometry examination. Results:BMD...
An association between adiponectin and renal dysfunction in patients with type 2 diabetes mellitus
A. Galovičová, I. Lazúrová, D. Petrášová
Vnitr Lek 2012, 58(11):825-829
Introduction:Adiponectin is a hormone of adipose tissue produced exlusively by adipocytes. According to recent studies its serum concentrations are negatively associated with obesity and diabetes mellitus type 2 (DM). The aim of the study was to clarify the relationship between renal functions and serum adiponectin in patients suffering from diabetic nephropathy. Patients and methods:The serum adiponectin levels were investigated in 120 patients and 40 controls. Patients in the diabetic group were divided according to albumin excretion rate into three groups - with normoalbuminuria (AER - albumin excretion rate/24...
Is polycystic ovary syndrome associated with autoimmune thyroiditis?
J. Petríková, J. Figurová, I. Dravecká, I. Lazúrová
Vnitr Lek 2012, 58(11):830-833
Introduction:Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathies in women of fertile age and lately there is a discussion about its possible association with autoimmune diseases. The aim of the studywas to examine incidence of autoimmune thyreoiditis (AIT) in PCOS women. Patients and methods:64 PCOS patients were enrolled and 68 healthy menstruating women served as controls. All subjects were examined for thyrotropin (TSH), free thyroxin (fT4) and the presence as well as titers of antithyroid antibodies aTG (anti-thyreoglobulin) and aTPO (anti-thyreoperoxidase). Results:There...

Multiple-combination kinezio-phlebothromboemboloprophylaxis, mechano-phlebothromboemboloprophylaxis and pharmaco-phlebothromboemboloprophylaxis of venous thromboembolism in internal medicine

Vnitr Lek 2012, 58(11):851-855
Parenteral heparin and oral coumarin preparations represent the standard antithrombotics widely used in the prevention and treatment of venous thromboembolism (VTE). Unfractionated heparin, low molecular weight heparins, and fondaparinux have been shown to be effective and safe in reducing VTE risk. Despite high efficacy these medicaments have limitations which become to be more apparent with current widening of the indications for either primary thromboprophylaxis (PTP) or indefinite and even lifelong secondary thromboprophylaxis (STP). There is a need for new effective and safe medicaments with specific antithrombotic action, oral administration...

Rheumatoid arthritis - an independent risk factor for cardiovascular disease
V. Ráček, P. Němec
Vnitr Lek 2012, 58(11):834-838
Mortality in patients with rheumatoid arthritis compared with general population is increased and approximately one-half of the premature death is due to increased cardiovascular mortality. Traditional cardiovascular risk factors only partly explain increased risk of cardiovascular disease in patients with rheumatoid arthritis. The risk of cardiovascular disease associated with rheumatoid arthritis equals that of diabetes mellitus. Rheumatoid arthritis should be regarded as a strong independent cardiovascular risk factor as diabetes mellitus. The presence of chronic inflammation implicated in pathogenesis of rheumatoid arthritis and atherosclerosis...
Vitamin D metabolism and current options for therapeutic activation of vitamin D receptor in patients with chronic kidney disease or renal failure
S. Dusilová Sulková
Vnitr Lek 2012, 58(11):839-849
Chronic kidney disease (CKD), and chronic renal failure in particular, is associated with vitamin D deficiency and with a disorder of all metabolic processes that are associated with vitamin D. Calcidiol levels are often low. At present, efforts are made to test and to pharmacologically modulate its levels and thus to contribute to greater availability of the substrate for external calcitriol production. Calcitriol production is reduced in CKD patients not only as a consequence of diminishing functional renal parenchyma but also as a consequence of 1-α-hydroxylase inhibition by FGF-23 and other factors. On the other hand, although parathormone...

Lenalidomide has been licenced for the treatment of multiple myeloma and, in 2012, it is used as a standard treatment of relapses of the disease. Literature contains a number of publications on the effects of lenalidomide in myelodysplastic syndrome, in malignant lymphomas and chronic B lymphocytic leukaemia. The effects of the drug in rare diseases, however, have not been investigated so far. In this paper, we summarize our experience with lenalidomide in rare blood disorders. We observed an excellent effect of lenalidomide in multifocal aggressive, repeatedly relapsing Langerhans cell histiocytosis where it led to complete remission. This patient...
Carcinoid and its cardiac manifestation
V. Talafa, D. Pastucha, K. Zeman, O. Zela, R. Filipčíková, M. Bezdičková
Vnitr Lek 2012, 58(11):867-870
A case report concerning patient diagnosed with a rare carcinoid showing typical symptoms is presented. Although essentially a cancer disease, this condition usually shows slow progression and tends to follow a favorable course with good prognosis if treated properly, assuming patient's reasonable compliance. We intend to highlight its typical clinical picture as well as the rarely occurring cardial manifestation leading to rapid progression and ultimately fatal outcome in a non-compliant patient who preferred alternative to evidence based medicine.
Coronary-subclavian steal syndrome, a complication following surgical revascularization of myocardium
V. Gloger
Vnitr Lek 2012, 58(11):871-874
Coronary-subclavian steal syndrom (CSS) is defined as a reversal of flow in a previously constructed internal mammary artery coronary conduit, producing myocardial ischemia. The most often cause is a proximal subclavian artery stenosis or closure. For the first time was CSS described in 1974 and initially was believed to be rare. However, today is the internal mammary artery to the left anterior descending coronary artery used in cardiosurgery as a standard and we can see increasing documentation of this phenomenon. A case report is presented and possibilities of management are discussed in the article.
Unusual cause of foot defect in patient with diabetes mellitus

Vnitr Lek 2012, 58(11):875-877
Foot complications are one of the most serious and costly complications of diabetes mellitus. Amputation of a part of a lower extremity is usually preceded by a foot ulcer. There have been also other not so typical causes of foot defect. The paper describe a history of a male diabetic patient with an atypical course of the foot defect.
Left ventricular myxoma - an unexpected cause of dyspnoea and fever in a young patient
T. Sattran, F. Toušek, L. Pešl, A. Mokráček
Vnitr Lek 2012, 58(11):878-880
Primary cardiac tumours are rare. In 75% of cases, these are benign, mainly myxomas. The present case study describes a patient with an impressive epidemiological history, examined at a department of infectious diseases for progressing dyspnoea and subfebrile states. As part of differential diagnosis, the patient was sent for cardiological examination. Echocardiography unexpectedly revealed tumour of the left ventricle that explained patient's symptomatology. The tumour was then quickly removed using right minithoracotomy and histology confirmed myxoma. The patient was, also due to the miniinvasive nature of the intervention, discharged from the hospital...
