Our experience in the treatment of membranous nephropathy with cyclosporine

M. Janeček, Z. Říhová, V. Tesař, M. Merta, D. Kmentová

Vnitr Lek 2005, 51(5):504-509

In a group of 17 patients with primary membranous glomerulonephritis, we assessed the impact of cyclosporine therapy on proteinuria and glomerular filtration. The indications for treatment with cyclosporine were: nephrotic syndrome resistant to chlorambucil (10 patients), cyclophosphamide (2 patients) or cyclophosphamide and chlorambucil (1 patient). We also treated with cyclosporine 3 patients with a high risk of thromboembolic complications (proteinuria above 10 g/24 hours) and one young patient was treated with cyclosporine at a dose not exceeding the gonadotoxic dose of cyclophosphamide. Treatment was started at a dose of 5 mg/kg/day and adjustments...

M. Belicová, J. Dvorský, Ľ. Vladár, M. Mokáň

Vnitr Lek 2005, 51(5):510-517

Clinical diagnosis of acute myocarditis is one of the greatest problems of contemporary cardiology, determined by wide clinical and laboratory polymorphysm of disease. Reliable, accurate diagnostic tests able to exclude or confirm diagnosis are not available yet. Aim of the study:Description of the prevalence, diagnosis and treatment of patients with acute myocarditis. Patients:From 1st July 2002 to 30th June 2003 12 patients (9 males) of age 24-49 years were hospitalised with the diagnosis of acute myocardidis. Results:8 patients had chest pain; these patients had changes on ECG. 4...

Prevention of venous thrombosis and pulmonary embolism in the department of internal medicine

J. Spáčil, J. Spáčilová

Vnitr Lek 2005, 51(5):518-522

From recent large controlled studies it is known that prevention of venous thrombosis and pulmonary embolism with heparin or low-molecular heparin in patients treated in the internal departments is indicated, effective and safe in all patients who have two and more risk factors for these disorders and who are not contraindicated. However, it is not known how the prevention is performed in practice, therefore we analysed clinical records of deceased with diagnosis of pulmonary embolism during two years and we studied clinical records of 411 patients hospitalised in the 3rd Medical Department of the 1st Faculty of Medicine and General...

Cause of clinical manifestations of chronic venous insufficiency in patients with overweight and obesity

D. Musil, J. Herman

Vnitr Lek 2005, 51(5):523-528

The aim of the paper was to answer the question to what degree clinical manifestations of chronic venous insufficiency in patients with overweight and obesity have connection with increased body weight, possibly with its various complications, and to what degree they are based on underlying primary infliction of lower limb venous system. Population consisted of 100 patients ((78 females a 22 males) with manifestations of chronic venous insufficiency (C1-C6 according to CEAP). Nutrition status of the patients was evaluated based on BMI: control group with BMI ≥ 20-25 kg/m2 (n = 49), monitored group with BMI ≥ 26 kg/m2 (n...

Urgent endoscopic papilosphincterotomy in individuals older than 70 years

P. Dítě, Z. Papík, M. Kunovská, I. Novotný, B. Kianička, P. Piskač, V. Zbořil, M. Zavoral

Vnitr Lek 2005, 51(5):529-532

Therapeutic endoscopy of acute pancreatobiliary conditions is represented in absolute majority of cases by endoscopic papilosphincterotomy performed after verification of finding in biliary duct and pancreatic duct by means of endoscopic retrograde cholangiopancreatography. Endoscopic operation is, of course, invasive performance that burdens the patients; therefore it is also accompanied with possible complications. In elderly individuals a risk factor of performance is undoubtedly polymorbidity, especially cardiovascular and bronchopulmonary diseases. In prospective study with individuals older than 70 years with acute cholangitis and acute biliary...

Portal vein flow is associated to central hemodynamics and biochemical signs of liver lesion in chronic congestive heart failure

E. Goncalvesová, P. Lesný, P. Chňupa, M. Luknár, J. Fabián

Vnitr Lek 2005, 51(5):533-538

Chronic congestive heart failure (CHF) is a syndrome with multiorgan manifestation. During chronic congestive HF changes in portal vein (PV) flow develop. Instead of normal continuous, pulsatile flow with higher diastolic velocity appears. Aim of the study was to describe PV flow characteristics in advanced CHF and compared them with parameters of central hemodynamics and biochemical sings of liver lesion. Patients and methods:74 pts (60 males) aged 47.2 ± 20.2 year with severe left ventricular systolic dysfunction (left ventricular ejection fraction 21 ± 4.1%) were evaluated. Portal vein flow was sampled from the main...

Catheter ablation of atrioventricular nodal reentry tachycardia - non invasive possibility of diagnostics, immediate and 1 year results following radiofrequency ablation and 1 year follow up of 40 patients treated in 2002

Y. Staňková, J. Müllerová, Z. Stárek, P. Vank, L. Zaoral, M. Novák

Vnitr Lek 2005, 51(5):539-547

Introduction:Catheter radiofrequency ablation (RFA) of atrioventricular nodal reentry tachycardia (AVNRT) is the method of first selection in patients with symptomatic tachycardia. Aim:Our target was to prove the yield of non-invasive investigative methods in the course of AVNRT diagnosis, to observe success of method and quality of life improvement after the RFA, X-rays stress, the number of RFA applications, time of skiascopy and effort time, complications of RFA and to compare exercise tolerance before and after the RFA. Patients and methods:40 patients with the diagnosis of AVNRT who underwent RFA from January...

Vasospastic angina pectoris - pathogenesis, diagnostics and treatment

E. Sovová, J. Lukl, Č. Číhalík

Vnitr Lek 2005, 51(5):548-554

In 1959 Prinzmetal described a syndrome of chest pains with typical ST segment elevation on ECG that is due to spasm of large epicardial or septal artery. Currently the term vasospastic angina (VSA) is used more often in literature. VSA can lead in acute myocardial infarction, it can be accompanied with serious cardiac arrhythmias and even sudden death is described in these patients. Pathogenesis of this syndrome is not completely clear. Effect of endothelial dysfunction, inflammatory factors, disorder of coagulation mechanisms, smoking and low level of magnesium belongs among suggested mechanisms of disease. ECG and Holter ECG monitoring is the basic...

Prolonged administration of low-molecular heparins in the prophylaxis of postoperative thrombosis

M. Čepelák

Vnitr Lek 2005, 51(5):555-559

Low-molecular-heparins have become a standard medication of venous thrombosis prevention in orthopaedic and general surgery. Reasons for extended prophylaxis with low-molecular heparins after hospital discharge are best evident in patients with total hip replacement. In this patient group a high incidence of thromboembolic complications in the late period after surgery has been ascertained and the origin of new asymptomatic thrombosis cases has been documented by phlebography and duplex sonography. A prolonged treatment with low-molecular heparins leads to a significant lowering of the frequency of manifest as well as asymptomatic thrombosis and finds...

Genetic tests in prediction of effectiveness and toxicity of chemotherapy in cancer patients

B. Mladosievičová, A. Fotlinová, M. Wawruch

Vnitr Lek 2005, 51(5):560-565

In few other fields of clinical practice is the role of pharmacogenetics and pharmacogenomics more important than in anticancer therapy. Genetic alterations in drug metabolism enzyme genes or genes encoding transporters and target proteins can affect not only the toxicity, but also response to commonly used chemotherapy drugs. Gene polymorphisms can influence drug effectiveness and/or toxicity in subgroup of patients treated with 6-mercaptopurine, thioguanine, irinotecan, methotrexate, 5-fluorouracil and platinum chemotherapy. Knowledge regarding single candidate genes polymorphism might be less helpful for predicting potential toxicity and effectiveness...

Pneumology problems of patients with diabetes mellitus

H. Bartáková, L. Brunerová, M. Anděl

Vnitr Lek 2005, 51(5):566-577

Diabetes mellitus is a system disease, which can result in a multiorgan dysfunction. The most serious pulmonary complications of diabetes mellitus are the lower respiratory tract infections developing under immunity dysfunction and under structural and functional pulmonary or extrapulmonary changes. Non-cardiogenic edema can occur as a complication of acute diabetic status such as ketoacidosis or hypoglycemic coma. Immunity dysfunction in diabetic patients seems to have even more complicated consequences according to a previously published study showing lower incidence of ARDS in diabetic patients with septic shock compared to nondiabetics. The question...

Obstructive sleep apnea, hypertension and erectile dysfunction

M. Pohanka, P. Kaňovský

Vnitr Lek 2005, 51(5):578-582

Erectile dysfunction and obstructive sleep apnea represent frequent, but generally underestimated phenomenon in clinical practice. Despite significant progress in understanding of erectile dysfunction pathophysiology, the mechanisms triggering onset and progression of organic erectile dysfunction are not completely known yet. Obstructive sleep apnea has several negative effects on the organism, causing autonomic, neurohumoral and endothelial dysfunction. There is an increased number of evidence that patients with obstructive sleep apnea may have increased risk for development of erectile dysfunction. This review will discuss potential pathophysiologic...

Vnitr Lek 2005, 51(5):583-587

Fifty-three years old woman was examined for severe gastrointestinal bleeding. She had positive history of recurrent gastrointestinal (GI) bleeding with enterorrhagia but the source of bleeding was not identified. The woman had severe anemia (hemoglobin 67 g/l), repeated blood transfusions were given. Gastroscopy, push-enteroscopy and coloscopy were negative. Therefore scintigraphy with red blood cells labeled in vivo by means of 99mTc was performed. Scintigraphy with red blood cells (RBCs) was positive for active gastrointestinal bleeding and showed spreading of the labeled RBCs into small bowel, the site of bleeding was detected...
