Pedal bypass in the treatment of diabetic foot ischemia - midterm results
P. Tošenovský, B. Zálešák, L. Janoušek, B. Kožnar, J. Peregrin
Vnitr Lek 2005, 51(2):163-166
Background:Unacceptably high percentage of diabetics undergo major amputation of their lower limbs annually. An occlusive aterosclerosis of crural arteries is major cause of amputations of those patients. The end stage of such process is development of ulcerations and gangrene of the foot. A progress in revascularisation techniques led to a more aggressive approach to solve this issue within last few years and specially methods of interventional radiology reached good results in the inrapopliteal region. However the scope of percutaneous techniques is limited and fail in defined number of patients. Pedal bypass is a good alternative for...
Pulse pressure in the young population detected by ABPM and it's relation to metabolic and anthropometric parameters
J. Pařenica, L. Pinková, O. Ludka, P. Fráňa, H. Němcová, J. Špinar
Vnitr Lek 2005, 51(2):167-174
Elevated value of pulse pressure (PP) is a predictor of increased cardiovascular mortality and morbidity. Ambulatory twenty four hour blood pressure monitoring (ABPM) shows a close correlation with end organ damage comparing with office blood pressure. The aim of this trial was to determine mean values of PP detected by ABPM in population aged 18-30 years, to determine variability of PP during day, to determine correlation between PP and systolic and diastolic blood pressure, heart rate and some risk factors of atherosclerosis. We have examined 252 probands (average age 23.6 ± 3.8, 55.2% were women). In this study the men population had significantly...

Vnitr Lek 2005, 51(2):175-182
Apoptosis failure during chemotherapy of leukemia is one of the multidrug resistance mechanisms. The aim of this study was to introduce in vitro model for malignant cell clone sensitivity testing. In vitro cytostatic treatment of acute myeloid leukemia (AML) and B-cell chronic lymphocytic leukemia (B-CLL) cells and its relationship to P-gp, MRP, LRP, Bcl-2, and Bax proteins expression was also investigated. AML cells were treated with doxorubicin and B-CLL cells were treated with chlorambucil. Flow cytometry was used for all experiments evaluation. To detect the amount of apoptotic cells in the sample three methods were used: annexin test, TUNEL (Tdt-mediated...

Geriatrics as an inter-disciplinary science on the threshold of the 21st century
P. Weber
Vnitr Lek 2005, 51(2):198-205
Author in his review refers to geriatrics as a peculiar scientific branch with a close relationship to internal medicine. Internal medicine is in fact a basic source for everyday clinical practice of geriatrician. However, there are some additional specific methods and approaches used while treating seniors. In addition to the basic characteristics of geriatrician's work the review highlights the importance of function examination in geriatrics, which, in addition to the physical evaluation, should stress the evaluation of self-sufficiency as well as mental and social status. Furthermore, the author analyses population age structure and associated...
Stem cells: new promise in medicine
M. Klabusay, P. Dvořák, J. Mayer
Vnitr Lek 2005, 51(2):206-215
The stem cells - both embryonic and adult - attract the attention of scientific and medical community nowadays. They could become a key to the treatment of serious diseases, because of their ability of the long-term self-renewal and differentiation. Many scientists around the world pay attention to stem cells research. This article gives a brief overview of the state-of-the-art of our knowledge about the human stem cells. In the introduction, the definition of stem cells and their nomenclature are described. In the chapter about embryonic stem cells, historical background, proof of pluripotency, and potential therapeutical applications are summarized....
Tumor lysis syndrome
A. Kafková, E. Tóthová
Vnitr Lek 2005, 51(2):216-220
Tumor lysis syndrome with hypeuricemia, also known as TLS, is a potentially serious complication of cancer chemotherapy and it can result in life-threatening complications such as renal failure and cardiac arrhythmias. The aim of this paper is to provide a general overview of the pathophysiology and corresponding signs and symptoms of TLS and review clinical and laboratory monitoring parameters. An understanding of the pathophysiology of TLS and meticulous assessment of signs and symptoms and monitoring of the laboratory data, as well as early recognition of problems are essential for optimal patient outcomes. Both allopurinol and rasburicase offer...
Adiponectin - the new adipocyte-derived hormone with relation to obesity and insulin resistance
J. Housová, D. Housa, M. Haluzík
Vnitr Lek 2005, 51(2):221-225
It is generally accepted that adipose tissue is an endocrine organ producing number of hormones. These hormones play an important role in the food intake regulation and significantly affect the development of atherosclerosis and insulin resistance respectively. Adiponectin - a recently discovered adipocyte-derived hormone - is one of the most extensively studied adipocytokines. There is a growing number of both experimental and clinical evidence suggesting that adiponectin is an important ethiopathogenic factor in the development of insulin resistance, diabetes and atherosclerosis. The aim of this article is to review the current state of knowledge...

Congenital adrenal hyperplasia, defect of 17α-hydroxylase as a rare cause of hypertension and hypocalaemia
V. Olšovská, J. Šulcová, A. Hondlová, H. Šiprová
Vnitr Lek 2005, 51(2):226-230
Authors describe infrequently occurring case of congenital adrenal hyperplasia, partial defect of 17α-hydroxylase in female patient with hypertension, hypocalaemia, menstrual cycle disorders and sterility. Diagnosis was established on the basis of native examination of steroid spectrum together with both stimulation and suppression tests and CT examination. After the treatment with dexamethason was established, the blood pressure and electrolyte levels reached the correction. This report supports the importance of the whole steroid spectrum examination in the patients with this symptomatology in order to eliminate rare causes of endocrine hypertension.
Heparin-induced thrombocytopenia in a patient with left main coronary artery stenosis
J. Hirmerová
Vnitr Lek 2005, 51(2):231-237
The article describes a case of a 75 year old female patient with hypertension and chronic atrial fibrillation, admitted for shortness of breath and chest pain. Coronarography revealed a significant stenosis of the left main coronary artery. The patient was indicated to coronary artery bypass grafting. During the stay, low molecular weight heparin had been applied. In the third week the platelet count fell down markedly and several days later, heparin necrosis developed in a place of subcutaneous heparin application in abdominal wall. Laboratory assays confirmed the presence of the antibodies against the heparin/platelet factor 4 complex. Thus the...
Treatment of chronic infection with hepatitis C virus with pegylated interferon and ribavirin in patient with mixed cryoglobulinaemia
P. Husa, Z. Adam, L. Husová, J. Feit, M. Penka, Z. Čermáková
Vnitr Lek 2005, 51(2):238-243
Mixed cryoglobulinaemia is the most common extrahepatic manifestation of hepatitis C virus (HCV) infection. Anti-HCV antibodies and ribonucleic acid of the virus, HCV RNA, can be detected in cryoprecipitates, together with rheumatoid factor. Cryoglobulins consist of complex of immunoglobulins that in vitro precipitate upon the cooling bellow the human body temperature. Vasculitis is caused by the deposition of such immunocomplexes in the small blood vessels. 36-45% of patients chronically infected with HCV have cryoglobulins in serum; however, less than 10% of them develop clinically manifested vasculitis. In the referred patient sporadic eruption...
