Late complications of chronic respiratory infections in patients with common variable immunodeficiency

Common variable immunodeficiency (CVID) is the most frequent serious humoral deficiency manifested in adulthood in the form of acute and chronic respiratory infections which in most patients lead to respiratory failure. Retrospective analysis of 28 CVID patients was made. Mean age at time of diagnosis was 38.6 ± 18.6 years. The time which elapsed from the first symptoms to determining the diagnosis was three times as long as that indicated in other studies, i.e. 14.1 ± 10.2 years. The importance of anamnesis in differential diagnosis of reflex and cardiogenic syncope

The aim of the study was to establish the difference in the anamnestic data in cardiogenic and reflex syncope during a systematic gathering of data using a structured questionnaire and to measure the benefits of structured clinical history in differential diagnosis of syncope. Methodology:70 patients with syncope of undetermined etiology (average age 51 years, 26 men and 44 women) underwent a standard diagnostic protocol procedure. Tako-tsubo syndrome - new addition in the family of acute states in cardiology. Current report

Tako-tsubo syndrome is most often described as temporary disorder in the kinetics in the apical part of the left ventricle. Typical of this state is pronounced dyskinesis, coupled in certain cases with intraventricular obstruction. The symptom can be accompanied by a dramatic clinical picture resembling myocardial infarction. However, the findings on the coronary veins tend to be normal and the patient's prognosis is favourable. To do a proper diagnosis, it is of primary importance to bear in mind the possibility of tako-tsubo syndrome.

We report a case of a 61-year old woman with progressive dyspnoea, fatigue and tiredness over the last two weeks. Based on physical examination on admission, ECHO and CT exams, a diagnosis of massive pulmonary embolism was confirmed and the patient received thrombolytic treatment. Since no response to thrombolysis was apparent and the patient was clinically unstable with persisting signs of right ventricular dysfunction in echocardiography, a rescue surgical embolectomy was ordered and performed using cardiopulmonary bypass.
