Ultrasound mapping of lower-limb vascular system with regard to occurrence and anatomy of additional front great saphenous vein
D. Musil, J. Herman
Vnitr Lek 2006, 52(12):1150-1155
A primary survey study involving a set of 113 lower limbs of 66 patients was focused on the prevalence of additional front great saphenous vein (AFGSV) and the measurement of the distance between its start in the great saphenous vein (GSV) and the saphenofemoral junction (SFJ), the measurement of the distance between the passage of AFGSV through the saphenous fascia and its start in GSV, and compared the width of AFGSV anechogenous lumen with the calibre of GSV. The prevalence of AFGSV in the observed set was 42.5%. Reflux was proved in 6 AFGSVs (12.5%). The width of the AFGSV anechogenous lumen was on an average 0.29 cm (M 0.26 cm, SD ± 0.15...
Prospective use of EuroSCORE for the short-term risk evaluation of consecutive cardiac surgery candidates: are there any differences in prediction of perioperative risk versus risk of nonsurgical treatments?
M. Klinčeva, P. Widimský, A. Dohnalová
Vnitr Lek 2006, 52(12):1156-1161
Background:In the era of modern medicine, the relevance of perioperative risk stratification with respect to the practice of cardiac surgeons and cardiologists worldwide has increased. Aim: This study aims at estimating the validity of a preoperative risk stratification model EuroSCORE in predicting short-term mortality in Czech adult cardiac population. Methods:All consecutive patients (n = 460) presented to cardiac surgeons as surgery candidates during a six-month period were enrolled in this study. The Additive and logistic EuroSCORE were used for the risk assessment. The operative mortality, as well as the mortality within...

Vnitr Lek 2006, 52(12):1162-1171
Background: Moderate and severe hemoptysis is a potential life-threatening condition which requires immediate medical examination and intervention. Aim: Retrospective evaluation of the effectiveness of bronchial artery embolization in the management of hemoptysis (over 50 ml per 24 hours) in the university hospital (from 1998 to 2005). Methods: A retrospective case study. Forty seven consecutive patients with hemoptysis over 50 ml per 24 hours were reviewed and data collected from medical documentation (medical history, chest X-ray, bronchoscopy, thorax spiral CT, pulmonary and bronchial angiography). Results: Forty seven patients,...
Allogeneic hematopoietic stem cell transplantation in patients with chronic myeloid leukemia in the Czech Republic - a retrospective analysis of results in years 1988-2005

Analyses of hematopoietic stem cell transplantation (SCT) results are of high importance for decision-making on treatment strategy for patients with SCT as a possible therapeutic alternative. In this paper the Czech National Registry of SCT and Transplantation Centre in Pilsen present their joint retrospective analysis of the results of allogeneic SCT in patients with chronic myeloid leukemia (CML) performed in the Czech Republic from 1988 to spring 2005. 295 patients (179 men and 116 women) ranging in age from 6.9 to 59.5 years (median 37.3) underwent transplants. In most cases the donor was an HLA-identical sibling (164; 55.6%) or a voluntary unrelated...

The electrocardiogram in acute myocardial infarction with reperfusion: current concepts regarding Q waves and their dynamics
J. Šochman
Vnitr Lek 2006, 52(12):1181-1184
The formation of Q waves in patients with myocardial infarction with primary ECG - documented ST elevations has been the subject of considerable attention. Less widely known, however, has been the fact that Q waves need not be a permanent feature and may disappear or decrease in quantity in some patients. For some time, the Q wave was perceived as a transmural extent of predominantly non-viable myocardial structures (scars or fibrosis). A most effective method for infarct-related artery recanalization is now available and the latest techniques have demonstrated beyond any doubt that even an invariable Q wave need not necessarily indicate loss of myocardial...
Non-arrhythmic therapy of ventricular tachyarrhythmias and sudden cardiac death after acute myocardial infarction
P. Schweitzer
Vnitr Lek 2006, 52(12):1185-1189
The management of ventricular tachyarrhythmias and prevention of sudden cardiac death after acute myocardial infarction (AMI) underwent important evolution. In the CAST study, encanaide and other antiarrhythmic drugs were not only ineffective but also increased mortality after myocardial infarction. Amiodarone had some beneficial effect on arrhythmic events without improving survival, and ICDs failed to improve outcome early after AMI. In comparison, short and long term survival benefits of beta blockers, angiotensine converting enzyme inhibitors and aldosterone antagonists after AMI is well established. This review discusses the role of non-arrhythmic...
Statins and osteoporosis
P. Švejda
Vnitr Lek 2006, 52(12):1190-1193
3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors, statins are potent inhibitors of cholesterol biosynthesis. Cholesterol-lowering therapy using statins significantly reduces the risk of coronary heart disease. Furthermore, wider use of statins leads to increases of other beneficial effects, so-called pleiotropic effects. These include reductions in cytotoxicity and inflammation reaction, oncoprotective effects - apoptosis of some types of cancer cells, improvement of endothel function, inhibition of thromb formation, decrease of the risk of dementia independent of improvement in the lipid spectrum, inibition of smooth muscle cells proliferation...
ZAP-70 in B-cell chronic lymphocytic leukemia: clinical significance and methods of detection
L. Smolej, L. Saudková, M. Špaček, T. Kozák
Vnitr Lek 2006, 52(12):1194-1199
B-cell chronic lymphocytic leukemia (B-CLL) represents a heterogeneous disease with highly variable prognosis. Clinical staging systems (Rai, Binet) fail to accurately predict the prognosis of individual patients, especially in early stages. Modern prognostic markers, mainly the mutational status of the variable regions of immunoglobulin heavy chains (IgVH) and genetic aberrations, allow more accurate risk stratification. Assessment of the expression of intracellular protein tyrosine kinase ZAP-70 represents not only a potential surrogate marker for the technically difficult and routinely unavailable assessment of the IgVH mutational status, but might...

Collagenofibrotic glomerulopathy - rare glomerulonephritis
G. Bernasovská, M. Demeš, A. Okša, M. Pavlovič, A. Vahančík, O. Nyitrayová, P. Gomolčák, D. Daniš
Vnitr Lek 2006, 52(12):1200-1204
Glomerulopathies with fibrillary deposits form a heterogeneous group of renal diseases that can be identified only by means of electron microscopy. A case of a rare type of such a nephropathy, the collagenofibrotic glomerulopathy with focus on differential diagnostics is presented and current knowledge relating to this renal disease is reviewed.
