Vnitr Lek 2005, 51(10):1070-1078
Focal atrial tachycardia is an infrequent arrhythmia in adult patients. The purpose of the study is presentation of clinical manifestation, localization of ectopic foci in the atria, and immediate and long-term results of catheter ablation. Patients and methods:Intracardiac mapping and catheter a ablation was employed in 50 patients (34 females), aged 44,9 ± 16,8 years, in 53 procedures. Results:A total of 50 ectopic foci, 36 in the right atrium or the inter-atrial septum, and 14 in the left atrium, were found and ablated. Noninducibility of the tachycardia at the end of the procedure was immediately...

The authors compared rertrospectively feasibility, efficacy, safety and mid-term results of transcatheter and surgical treatment of isolated ostium secundum atrial septal defect in adult patients. Transcatheter closure was performed in 32 patients and surgical closure in 39 patients. Both methods were well matched in feasibility (97 % vs 100 %; NS) and in the incidence of major complications (0.0 % vs 2.6 %; NS). There were fewer minor complications (9.4 % vs 59.0 %; p < 0.001) and shorter hospital stay (3.3 ± 1.3 days vs 12.6 ± 4.5 days; p < 0.001) but higher costs (206 ± 22 vs 129 ± 55 thousand Czech Crowns; p <...

Vnitr Lek 2005, 51(10):1087-1094
The work analysed the incidence of hypertension and control of hypertension in offices of 150 general practitioners. A survey was carried out using questionnaires. Each of 150 general practitioners enrolled 15 consecutively examined patients into the study. Only patients at the age of 45 years and above without age limit in the elderly age groups were enrolled. The entire population thus consisted of 2 211 individuals, of which 1 012 (45.8 %) were men and 1 195 (54.0 %) women. Hypertension was defined as blood pressure ≥ 140 and/or ≥ 90 mm Hg or as the presence of antihypertensive treatment. The study demonstrated high prevalence of hypertension...
Prevalence and associated factors of self medication with antibiotics in Czech Republic

Vnitr Lek 2005, 51(10):1096-1101
Self medication with antibiotics (ATB) in Czech Republic (CR) is not approved by law. During the past era a very strong ATB policy and strict control of ATB use in the CR. The objectives of this study were to determine prevalence and to identified factors related of actual self medication with antibiotics and 'at risk' self medication with antibiotics. Method:crosss sectional design, two stage sampling, selecting a representative region, a middle size city (urban) and rural area sampling. Self administered postal questionnaire were randomly sent to to adults age ≥ 18, 1 000 respondents in urban and 1000 in rural area. Descriptive,...
Erythropoietin in the treatment of patients with myelodysplastic syndrome
J. Čermák
Vnitr Lek 2005, 51(10):1102-1107
Aim:The aim of the study was to evaluate the indication of erythropoietin in patients with myelodysplastic syndrome (MDS). Methods:Twenty patients with MDS of RA or RARS subtype (according to the FAB classification) were treated with recombinant human erythropoietin (rHuEPO) in the dose of 150 IU/kg 3 times per week for 12 weeks, in 1 patient with profound granulocytopenia rHuEPO was combined with G-CSF in the dose of 300 μg 3 times per week. The criteria for complete response (CR) were achievement of Hb ≥ 115 g/l and transfusion independence, increase in Hb ≥ 15 g/l and transfusion independence were evaluated as...

Vnitr Lek 2005, 51(10):1131-1137
Chronic hepatitis B is one of the most frequent infectious diseases on the world scale. Its prevalence in the Czech Republic is 0.56 %. Anti-virus treatment of chronic hepatitis B clearly improves the quality of life and is undoubtedly less expensive than the treatment of complications of advanced hepatic cirrhosis or hepatocellular carcinoma, provided the indication criteria and standard therapeutic procedures are applied. Pegylated interferon (IFN) α-2a represents the most effective treatment currently available and should be therefore a first choice remedy in the initial therapy of both HBeAg positive and negative chronic hepatitis B, unless...

The importance of L-carnitine in patients with chronic renal failure treated with hemodialysis
R. Cibulka, J. Racek, E. Veselá
Vnitr Lek 2005, 51(10):1108-1113
In patients with chronic renal failure dysfunctions of metabolism occur because of a basic affection and also of the hemodialysis. These dysfunctions result in a rise of serious complications for patients in the course of praedialysis period and also in the course of dialysis treatment. The population of hemodialysis patients is therefore a very endangered group of persons. They are in danger of early atherosclerosis, hyperparathyroidism, anemia and other diseases. L-carnitine is a necessary substance for transport of long chain fatty acids across the inner mitochondrial membrane so that it enables their β-oxidation and obtaining of energy,...
The role of apoptosis in cardiovascular diseases
P. Hude
Vnitr Lek 2005, 51(10):1114-1119
Active cell death (apoptosis) represents a genetically predetermined way of cell decay. It is therefore a counterpart to tissue proliferative activity. The integrity of a multicellular organism is determined by dynamic balance between the production of new cells and the decay of other cells. This allows to maintain an optimal number of cells in tissues with a different proliferative activity throughout ontogenesis and in postnatal life. Cell death thus becomes a normal and essential part of morphogenesis. The significance of apoptosis in some cardiovascular diseases is described: hypertension, ischaemic heart disease, heart failure, cardiomyopathy...
Prothrombotic status as part of metabolic syndrome
A. Remková
Vnitr Lek 2005, 51(10):1120-1125
Metabolic syndrome is associated with prothrombotic status, which participates in the development of atherothrombotic complications. Nevertheless, dysregulation of haemostasis is of a complex nature and involves endothel activation, hyperactivity of thrombocytes, hypercoagulability and hypofibrinolysis. Increased expression of type 1 plasminogen activator inhibitor with subsequent hypofibrinolysis is the main haemostatic disorder in relation to insulin resistance and is currently considered as a part of a set of abnormalities within the scope of the metabolic syndrome. The mechanism which leads to prothrombotic status in metabolic syndrome is multifactorial....

Prosthetic Mitral Valve Endocarditis
H. Pavelčíková, J. Ondrášek
Vnitr Lek 2005, 51(10):1126-1129
Infective endocarditis is still serious disease. Without early diagnosis and treatment it has very poor prognosis. Prosthetic valves represent one of the most important risk factor for development of infective endocarditis. The author is presenting a case report of the 56-years old woman with prosthetic endocarditis after skin ulceration treatment without right antibiotic prophylaxis.
